Step 3 · Contact details

Appointment request details

Provide contact details for an administrative appointment enquiry. Your preferred service, day and time are included with the request; the clinic must confirm availability.

Do not include symptoms, prescriptions or health records.

    Step 4 · Supporting documents

    Referral or supporting document

Uploads are disabled in this demonstration. A clinic must provide its appropriate contact route for health records.

    Before requesting an appointment

Read the relevant service page and decide whether you need a first consultation or a follow-up. If another professional has referred you, keep the referral available so that the clinic can confirm the correct service. Different appointment types may have different durations and fees.

Tell the team about access or communication arrangements when the visit is being organised. This could include step-free access, an interpreter or attending with a support person. Do not assume that a particular arrangement is available until the branch has confirmed it.

For a real booking, the confirmation should state the service, date, time, location and fee, together with any preparation instructions and cancellation terms. Keep those details available when you travel.

    Questions

    Booking information.

    Which appointment should I choose?

Start with the service directory. It explains the purpose of each appointment type and the questions to clarify before booking. If you have a referral, use the service named by your referring clinician. The general enquiry form is for administrative questions only.

Can I arrange more than one service on the same day?

A real clinic may coordinate separate appointments, but this depends on the service, branch and availability. Confirm each appointment individually and leave enough time between them. The times displayed in this demo are not live bookings.

What should I bring to a first visit?

Bring the documents listed in your appointment confirmation, any relevant referral or reports you have been asked for and a list of the questions you would like to discuss. Keep sensitive records off the demonstration enquiry form.

    How are fees explained?

The fees page shows a sample schedule and distinguishes consultations from separately arranged services. A real clinic should confirm the fee and what it includes before you agree to the appointment. Any change in scope should be discussed with you.

    Can someone come with me?

Ask the branch about its arrangements for a parent, carer, interpreter or support person. If you need help communicating or getting around the building, explain this when arranging the visit so that the team can confirm what is available.

How do I change an appointment?

For a real booking, use the contact details in your confirmation. Tell the clinic which appointment needs to change and ask about any notice period or cancellation terms. This website does not hold appointments, so the example selections cannot be cancelled or confirmed.
